An Aneurism in the Right Popliteal Artery

Abstract

Introduction: Popliteal artery aneurysm appears more frequently in the male sex between 65 and 80 years of age. Several associated etiological factors are described, among which are age, diabetes mellitus, arterial hypertension, smoking and dyslipidemia.

Objective: To present the case of a patient with an aneurysm of the right popliteal artery and who underwent surgery with prosthetic replacement of the artery.

Methods: The case is presented of a 58-year-old male patient of white race, with a history of hypertensive heart disease and type II diabetes mellitus; in addition, he had double valve replacement with mechanical prosthesis due to aortic and mitral valve insufficiency. In 2018, he was diagnosed with an aneurysm of the left popliteal artery, and underwent surgery at Hospital Hermanos Ameijeiras. For 18 months, he had been presenting an increased volume of a tumor mass on the right leg and with pulsatile characteristic, color change, intense pain and the impossibility to walk, relieved with rest.

Results: The patient was operated on under general endotracheal anesthesia/total intravenous anesthesia. During the operation, he underwent aneurysmectomy with terminal prosthetic graft using 7 mm polytetrafluor.

Conclusions: After adequate anesthetic recovery, assessed by the modified Aldrete scale, the patient presented posterior tibial pulse in the right lower limb and was transferred to the intensive care unit.
