Integration of health care levels in the management of diabetic foot in Mayabeque

Abstract

Introduction: The diabetic foot is associated with high hospital expenses. In 2019, in a health area in the Mayabeque province, some weaknesses in care were identified; and a group of strengths were recognized: it has been suggested that, despite the severity of the injury, the patient being well compensated, hospitalization is not necessary; In Cuba, profound changes have been made in Primary Care related to their care; among other.

Objective: Describe the results obtained with the implementation of the new document prepared for diabetic foot management in the consultation.

Methods: A retrospective study was carried out in 61 patients treated for diabetic foot in angiology between 2019 and 2020.

Results: 41 had Wagner lesions ≥ 1. 69 outpatient surgeries were performed. 27 patients presented complex injuries: 26 were offered frequent outpatient hospital care, combined with care at the primary level, through three modes of care with the use of information and communication technologies and admission to the home; one patient was hospitalized with a stay of 14 days and Heberprot-P® was applied in all of them.

Conclusions: With the diabetic foot management that was implemented, satisfactory care was offered to patients who presented complex lesions.
